The staff in social services and discharge planning provides services
to facilitate a continuity of care and treatment for patients during
their admission to Grace Cottage, their hospital stay, their discharge
and aftercare. Referrals are accepted from any source and can be written

At Grace Cottage Hospital, the patient is seen as a total person with
physical, social, spiritual and emotional needs that can have an influence
on a their recovery. The staff helps the patients and their family members
use healthcare services and their own resources in treatment of an illness
by working directly with the patient, family, hospital staff, and other
agencies and resources in the community.
