Townshend

The Referral Coordinator promotes and manages referrals, assesses and facilitates intake of patients to Grace Cottage Hospital. The Referral Coordinator has experience with the continuum of care and criteria for admission. The Referral Coordinator possesses expertise to assure that at each point in the patients course of treatment care is delivered in the optimum setting, from the perspective of clinical appropriateness, as well as from the resource utilization perspective assuring the coordination and efficacy of health care services.
The Referral Coordinator assesses patient needs (financial, emotional, medical, psycho-social) along the continuum of care. He/she assists in referrals for and acquisition of needed resources, and provide support and guidance to families and patients regarding illness management strategies The Referral Coordinator acts as an effective liaison with regional hospitals in order to facilitate patient transfers. Initiates and develops referring physician outreach activities in order to increase inpatient referrals. Analyzes referral patterns and trends in local markets and creates a strategic sales plan to develop new referral sources Marketing, clinical experience and proven results meeting utilization goals are preferred.
